The orthopedist thinks I have Fibromyalgia. I will be re-researching it 
                later, but from past research the best starting points are

                Anyway, I see a rheumatologist in early August. Have to call my PCP and 
                ask for pain killers. The Arthritis site says oh, fuck, I'm already 
                taking 3 of the recommended drugs. No they don't say that, I say that. I 
                guess I just drink tonic water until August.

There is an excellent section on phototherapy in
                Goodwin and Jamison's 
                Manic-Depressive Illness. Salient info:
                * In early studies patients were exposed to 2,500 to
                3,500 
                lumens. This level required at least 2 hours of
                treatment a day, and as 
                much as 4 hours. At the time of writing no specific
                effective frequency 
                had been determined, but the normal fluorescent bulb
                they were using worked 
                just fine.
                * Some patients reported eye strain, headaches and
                the like from having 
                even that low level of fluorescent light shone
                directly in their 
                eyes. These symptoms decreased by increasing the
                distance to the light, 
                i.e. reducing the intensity. There is no evidence of any permanent eye 
                problems.
                * Phototherapy should be administered just after dawn or just before 
                dusk so that it doesn't change the pattern of melatonin secretion.
                * Melotonin supplements can reproduce the symptoms of atypical 
                depression but don't necessarily bring on relapses of depression.
                * ELF (extra-low frequency electromagnetic) fields may disrupt the 
                circadian rhythms. Note that there are new therapies professing to prevent 
                disease by applying ELF fields.
                * Bipolars tend to be supersensitive to light and it may even be a 
                trait marker. Lithium reduces this.
                * Ambient temperature is also a factor both in SAD and in summer 
                hypomania. It seems that light level creates the tendency toward a mood 
                swing, but the temperature triggers it.


                My own experience and opinion:
                * My home-made array provides 5,000 lumens vs. the commercial units 
                which run as high as 10,000 lumens.
                * I start on Nov. 1 every year.
                * Two hours in the morning makes me more energetic. I have to work up 
                to that slowly to avoid triggering hypomania.
                * In January I require more light.


                Of course I don't follow my own advice. I stopped using the lamp after a 
                couple of days and over the weekend I crashed. I'll get into my own issues 
                in another email.
